We have two easy options for getting in with our doctors: Online Scheduler or via phone. We have easy to access Online Paperwork as well!
Online Scheduling and Paperwork
New Patients
- Step 1: Schedule an “Exam: New Patient” with Dr. Nelson using our Online Scheduler. If you have any questions about new patient appointment availability, please call our office at 763-420-4242.

Step 2: Fill out some paperwork using our Online Paperwork. Here’s what you need to fill out:
- New Patient Intake Form
- HIPAA Consent
- Note: if this appointment is related to an open auto claim, also fill out: Motor Vehicle Accident Questionnaire
- Step 3: Plan on showing up 15 minutes prior to the appointment time, we’ll need to make a copy of your drivers license and insurance card and get a few things in order before you see our doctors at your designated appointment time.
Existing Patients

Current patients
- Book your “Office Visit” or "Acupuncture" appointment with Dr. Nelson or Dr. Johnson using our Online Scheduler.

Over 1 year since your last appointment OR you are an existing patient that has been involved in a motor vehicle accident or workplace accident.
- Schedule an “Exam: Current Patient” with Dr. Nelson or Dr. Johnson using our Online Scheduler.
- Plan on showing up 5 minutes prior to your appointment time.
- Note: if this appointment is related to an open auto claim, also fill out: Motor Vehicle Accident Questionnaire.

Over 3 years since your last appointment
- Schedule an “Exam: 3+ Years Since Last Appointment” with Dr. Nelson or Dr. Johnson using our Online Scheduler.

Fill out some paperwork using our Online Paperwork. Here’s what you need to fill out:
- New Patient Intake Form

If you have questions or would prefer to schedule over the phone, call (763) 420-4242 to speak with one of our staff members. We are happy to offer paper copies of our intake paperwork, but please allow extra time if you plan to fill it out in our office in order to give our doctors the full appointment time to work with you.
